Black Ops brand 1" quill stem.  Here's a classic cast quill stem design that was used alot during the 80's, mainly by Suntour, Redline, and Schwinn.

4-bolt top load design.  Fits all old-school BMX bikes, and most other bikes that use a 1" threaded (not mini) headset / fork combo.
